What you would be doing: As a member of the interdisciplinary team, the Social Worker provides services to respond to the emotional needs of residents and their families from admission to discharge. You will conduct assessments, assist with admissions, transfer discharge and other paperwork, provide, or refer to proper support services, monitor and report changes to team members and family as appropriate.
