In this role, you'll provide compassionate care to patients in their homes during a sensitive and important time in their lives.

What You'll Do:

Provide High-Quality Care: Perform patient assessments, develop and implement individualized care plans, and collaborate with the interdisciplinary care team.
Educate and Support: Teach patients, families, and caregivers about patient care techniques and provide ongoing support.
Ensure Quality of Care: Monitor patient progress, document care effectively, and communicate with the care team regularly.
Advocate for Patients: Advocate for patient needs and ensure they receive the highest quality of care.
On-Call Responsibilities: Participate in on-call rotations to provide support to patients and families as needed.

Competitive Compensation: $88,000 - $92,000 per year + $10,000 sign-on bonus + mileage reimbursement.

What You'll Bring:

Nursing Experience: One year of nursing experience.
Current Licensure and Certification: Current RN license in the state of Wisconsin and current CPR certification.
Valid Driver's License: Reliable transportation and valid driver's license with insurance.
Compassion and Empathy: A genuine passion for providing compassionate and supportive care to patients and their families.
