Job Description

Ovation Primary Care & Hospice , a Stellar Senior Living Company, continues to grow and we are now expanding in the Arizona market! We are currently seeking a highly skilled and compassionate Primary Care Provider / Nurse Practitioner with a focus on geriatrics, hospice, and palliative care to join our primary care team. As a Primary Care Provider / Nurse Practitioner with Ovation, you will play a vital role in delivering comprehensive healthcare services to our elderly population. What we offer:

Responsibilities:

Provide high-quality primary care services to geriatric patients, focusing on promoting healthy aging and managing chronic conditions.
Conduct thorough assessments, perform physical examinations, and order and interpret diagnostic tests.
Develop and implement individualized care plans based on patient needs, including medication management, preventive screenings, and health promotion activities.
Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure holistic and patient-centered care.
Assist in coordinating and providing hospice and palliative care services, including pain management, symptom control, and emotional support for patients and their families.
Educate patients and their families about disease management, treatment options, and end-of-life care decisions.

Requirements:

Current and unrestricted Nurse Practitioner license in Arizona.
Minimum of 2 years of experience as a Nurse Practitioner, with a focus on geriatrics, hospice, or palliative care.
Strong knowledge of geriatric healthcare, chronic disease management, and end-of-life care principles.
Excellent clinical assessment and diagnostic skills.
Ability to work collaboratively in an interdisciplinary team environment.
Exceptional communication and interpersonal skills, with a compassionate and empathetic approach to patient care.
Ability to travel in the Phoenix area
