Primary Care Nurse Coordinator (RN) drives patient-centered care coordination in an outpatient family medicine/internal medicine clinic, optimizing access, quality, and outcomes through evidence-based protocols and proactive population health. Daily responsibilities include triaging patient calls and portal messages, conducting nurse visits and chronic disease management (hypertension, diabetes, COPD, asthma), performing medication reconciliation and immunizations, closing care gaps for preventive screenings and annual wellness visits, coordinating referrals, diagnostics, and prior authorizations, preparing and rooming patients with vitals and education using motivational interviewing, managing transitions of care after ED/hospital discharge with timely follow-up, executing outreach and panel management for value-based care metrics, supporting telehealth and remote monitoring workflows, and maintaining accurate EHR documentation, registries, and quality reporting (HEDIS, UDS). The ideal candidate is a licensed RN with ambulatory or primary care experience, exceptional clinical assessment and communication skills, EHR proficiency, and a collaborative approach with physicians, APPs, social workers, and pharmacists to advance safety, health equity, patient experience, and care management goals.
