Job Description

At Inbound Health, we provide an enablement platform with full-stack capabilities and proprietary technology and analytics tools customized for hospital and skilled nursing at home. Our flexible partnership structure helps customers leverage their existing assets while filling the necessary gaps for program scalability. With over 6,000 patients served across 350+ primary diagnoses since May 2020, we enable our clients to deliver safe, coordinated, and patient-centric care. Our comprehensive approach integrates biometric monitoring, in-home nursing, virtual visits, and a reliable supply chain, prioritizing quality and safety.

The Full-Time Clinical Liaison serves as the face of Inbound Healths Hospital-at-Home program and liaison between IH and hospital partners and patients. Provides patient care coordination for Hospital-at-Home to ensure continuity of care for program patients. Helps to identify qualified patients for Hospital-at-Home  and CMS waiver program. Lead and coordinate patient care activities. Work
collaboratively with internal and external partners/stakeholders to promote and cultivate the program.

RESPONSIBILITIES :

  • Actively promote Inbound Hospital-at-Home and CMS waiver programs to healthcare providers, establishing and maintaining relationships with physicians and healthcare professionals, cultivating a strong referral network.
  • Conduct comprehensive clinical assessments in collaboration with patient care team to identify and determine appropriateness for patient participation in Inbound Hospital-at-Home and CMS waiver programs.
  • Partner with patient case manager to create smooth patient transfer plans, ensuring continuity of care, and assist in removing barriers to getting appropriate patients.
  • Participate in quality improvement initiatives to enhance the delivery of patient care and promote best practices in care coordination and transitions of care.
  • Educate and counsel patients and their families about Inbounds programs and respond to patient questions and concerns.
  • Support Utilization Management efforts.
  • Maintain accurate and up-to-date records of patient assessments, care plans, and communication with healthcare providers and families. 
  • Adhere to all relevant healthcare regulations, compliance standards, and Companys Code of Conduct.
  • Stay abreast of current trends, developments, and regulations in healthcare delivery and care coordination.
  • Participate in special projects. 

EDUCATION:

  • Associates or Bachelors Degree in Nursing.  

CERTIFICATION REQUIREMENTS:

  • Current licensure as a registered nurse (RN) or Licensed Practical Nurse (LPN).
  • Additional certifications are plus, but not required.

WORK EXPERIENCE REQUIREMENTS :

  • Minimum of 2-5 years of clinical experience in a healthcare setting, with demonstrated expertise in care coordination, discharge planning, and patient advocacy. 
  • Experience in Emergency Department, Home Health nursing, and acute care (adults) highly desired.
  • Previous experience as a liaison or similar role in health industry. 
KNOWLEDGE & SKILLS :
  • Knowledge of healthcare regulations (including Stark Laws), reimbursement policies, and quality improvement initiatives related to care transitions and care coordination.
  • Proficient in electronic health/medical record systems and other healthcare software applications used for documentation and communication. 
  • Adaptable learner, comfortable with new technologies.
  • Strong communication (verbal/written) and interpersonal skills.
  • Skilled in care coordination & discharge planning.
  • Knowledge of medical terminology, procedures, and treatments.
  • Strong networking abilities with the ability to build rapport quickly.
  • Comfortable initiating and leading conversations with diverse groups of people.
  • Strong understanding of patient experience, roles and responsibilities within a hospital setting.

ADDITIONAL DETAILS:
  • Hours include required holidays & weekends.
  • Hybrid optional after 90-days of employment

Hourly Pay Range: $40.00-$45.00. Individual compensation is based on skill set, experience, qualifications, location, labor markets, and other job-related reasons.
