Job Description

Are you a dedicated Registered Nurse with a passion for providing top-notch care management? We are looking for experienced RN Care Managers to join our team for a remote opportunity serving managed care plan members. This position is fully remote after an initial 2-3 weeks of in-office training in Manhattan, NY. We are seeking candidates who have a New York State RN License and prior experience in Managed Long-Term Care (MLTC) settings.

Key Responsibilities:

Provide clinical support and oversight to ensure members receive safe and effective care within the community.
Become proficient in Article 49 regulations.
Manage service authorizations, create care plans, and coordinate services across the continuum of care for high-risk populations.
Assess and coordinate services for hospitalization, skilled services, home care, and nursing home care.
Oversee coordination of benefits for Medicare, Medicaid, and other insurance providers.
Facilitate transitional care management, including transitions between home, skilled nursing facilities, and hospitals.
Conduct monthly call assessments and follow up on changes in medications, diagnoses, or significant events like ER visits or falls.
Coordinate post-discharge services and perform other duties as assigned.
Qualifications:

RN (BSN preferred) with a current, valid, and unencumbered license, certificate, or registration.
Experience as a New York licensed RN in home care, acute, sub-acute, long-term care, or managed long-term care settings.
Ability to maintain all state and federal compliance requirements for licensure and employment.
Bilingual skills (Spanish, Cantonese, Mandarin) and UAS certification are a plus.
Knowledge of care management preferred.
Must reside in Downstate NY, Westchester, Long Island, or Northern New Jersey and be able to commute to Manhattan for training and occasional in-office meetings if needed.
