Join Our Team in Population Health and Collaborative Psychiatry in multiple clinic locations across NYC. We are seeking a dedicated Psychiatric Mental Health Nurse Practitioner or Physician Assistant to join our organization, providing integrated psychiatric care for geriatric patients. This role is ideal for a practitioner who thrives in a collaborative, multidisciplinary environment and is passionate about population health and psychiatric-integrated primary care. You will focus on managing sub-acute and acute psychiatric care, ensuring seamless transitions back into community-based care after a 3–6 month follow-up period.
Compensation & Benefits:
Base Salary: $170,000 - $190,000 plus annual bonus.
Full benefits package including health insurance, retirement plans, CME allowance, and PTO.
Role Overview:
Manage medication management for geriatric patients within the PACE program.
Provide psychiatric care for patients requiring short-term stabilization (3-6 months) before transitioning back into community support.
Collaborate with primary care providers and multidisciplinary teams to ensure holistic patient care.
Work within a managed care model to integrate behavioral health services with primary care.
Conduct psychiatric evaluations and follow-up visits in outpatient clinic settings with some telemedicine.
Coordinate care plans with social workers, nurses, Psychiatrists, primary care providers and other healthcare professionals.
Focus on psychiatric management, this position does not involve therapy.
Contribute to improving population health outcomes through proactive psychiatric interventions.
Requirements:
Board-certified Psychiatric Mental Health Nurse Practitioner or Physician Assistant
2 years of experience as Psychiatric Provider
Experience in geriatric psychiatry, population health, or integrated primary care preferred.
Strong ability to work within a multidisciplinary, team-based approach.
Experience in managed care settings is a plus.
