We’re looking for a dedicated and compassionate Psychiatry Nurse Practitioner (PMHNP) to join our team. We’re looking for a Psychiatric Mental Health Nurse Practitioner (PMHNP)—either an experienced clinician or a recent graduate—who is passionate about geriatric psychiatry and dedicated to making a positive difference in the lives of elderly patients.
In this role, you will offer direct psychiatric care to residents in nursing and rehabilitation facilities throughout New York County. Working as part of an interdisciplinary team, you will perform evaluations, manage medications, and support patients’ mental health through evidence-based interventions. Perform comprehensive psychiatric assessments and create individualized treatment plans.
Prescribe and manage psychotropic medications, adjusting as clinically necessary.
Provide therapeutic interventions and supportive counseling when appropriate.
Partner with physicians, nurses, psychologists, and social workers to coordinate care.
Educate patients, families, and facility staff about mental health conditions, treatment plans, and wellness practices.
Complete accurate, timely documentation in the electronic health record (EHR).
Participate in team meetings, clinical case discussions, and quality improvement projects.
Active New York State Nurse Practitioner license and PMHNP certification .
Ability to work effectively both autonomously and within a multidisciplinary team.
Excellent interpersonal and communication skills with a compassionate, patient-centered approach.
Experience with EHR systems and general computer proficiency.
Competitive annual salary ($130,000–$170,000, DOE).
