Franciscan Alliance Inc. d/b/a Franciscan Physician Network seeks Physician at its facility located at 1710 Lafayette Road, Crawfordsville, Indiana 47933. To provide advanced care and specialty physician services within the hospital. Responsible for patient health assessment and create strategies to improve or manage patient health. Include strategies which include managing the entire treatment process from admission to discharge. Recommend treatment plans, record medical histories. and order tests to diagnose and treat the patient. Responsible for updating information of existing patient records with the most recent findings. Introduce habits that promote health into a patient's life. Responsible for managing the entire treatment process, that is, acute, chronic, or emergent for patients admitted to the facilities.
Provides Internal medicine care to adult patients in a small community hospital setting in rural Indiana. Provides Acute and/ or Critical medical care to hospitalized patients in rural area. Diagnosis and treats of various illnesses and diseases within a hospital setting. Renders care coordination with specialists, nurses, patient's primary care provider and other healthcare professionals. Makes daily rounds to assess the condition of patients, order tests, review results, adjust treatment plans and communicate with patient and their families. Serves on hospital and medical staff committees to help improve efficiencies, patient care, safety, infection control, and other quality improvement efforts. Responds to questions and concerns from the patient and patient family regarding the care and treatment plan. Educates patients on new medical conditions to help promote good health and quality of life. Maintains stays abreast on the latest medical treatments and medical care advancements. Collaborates with advance practice providers within the hospitalist service. Provides care for patients in urgent and emergency medical situations within the hospital such as code blue, rapid response chest pain, or stroke alert. Performs various patient procedures within the scope of internal and hospitalist medicine. Also maintains detailed medical records within the electronic medical record system and ensures that all patient information, diagnoses, and treatment plans are accurately documented.
REQS: The position requires an MD or DO degree. The employer will accept ECFMG equivalency to MD. Additionally, the applicant must have: (1) Completed a residency in Internal Medicine from an accredited institution. (2) Licensed to practice Medicine in IN; and (3) Board Certified or Board Eligible in Internal Medicine.
