Job Description

**$5,000 SIGN ON BONUS for Full time field clinicians**

Central Coast Home Health is a fast-growing and locally owned organization with an excellent reputation throughout San Luis Obispo and Santa Barbara counties. We are constantly growing and now seeking a Full time Palliative Care Case Manager to join our team in the Santa Maria, Lompoc and Santa Ynez Valley area.

Palliative Care Nurse  cares for patients with life-limiting diseases who are still seeking curative treatments and interventions. The focus is on symptom management, advance directives and other end of life decisions, and education regarding disease processes. The Palliative Care nurse works collaboratively with a multidisciplinary team including social workers, spiritual counselors and physical/speech/occupational therapists. By seeing your patients in the home, you are able to spend the time necessary developing relationships, answering questions and assessing the needs of the patient and family on a psychosocial, physical and spiritual level. 

Regular work schedule is Monday through Friday, 8 AM- 5 PM with a choice between 4 or 5 days/week and a weekend rotation approximately every 4 weeks.

Job Requirements:

  • Current California RN License
  • At least one year of professional nursing experience
  • CPR certified (Healthcare Provider)
  • Home Health and/or Hospice experience is a plus
  • Reliable vehicle to drive for work
  • Current Driver's license and clean driving record
  • Have and maintain auto insurance coverage

  1. Comfortable working independently in a home-visit setting
  2. Must be able to effectively communicate with elderly and chronically ill patients and families

This position is paid by the visit. Compensation for routine visits ranges from $54.00-$61.00 per visit; evaluations range from $92.00-$104.00 per visit; and admissions range from $162.00-$183.00 per visit. Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
