Job Description

The individual in this position performs psychosocial assessments and develops plans to meet patients' needs with complex social issues. They also arrange post-discharge services as needed.

Responsibilities include:

• Assisting with adoptions, abuse, and neglect cases, and making referrals to agencies.
• Participating in weekly Complex Case Review meetings and daily patient care conferences.
• Providing resources and referrals to community agencies for ongoing counseling or care.
• Maintaining an updated list of post-discharge care providers.
• Communicating with post-discharge providers.
• Updating Case Managers on changes to post-discharge plans.
• Monitoring and reporting to the DCM and hospital leadership.
• Staying informed about regulatory requirements and community resources.
• Performing other duties as assigned.

Under the general direction of the Social Work Manager/LCSW, this role delivers age-appropriate social work care to patients and caregivers following hospital policies and regulations. The Social Worker II is an essential part of the healthcare team, providing assessments, coordination, treatment planning, and referrals to meet patients' and families' complex needs in hospital and clinic settings. The MSW will seek supervision from a Licensed Clinical Social Worker as needed and will use the Patient Medical Record and HPF systems.
