Job Description

The Position

Under the general supervision of the Health Manager, the Community Health Nurse will embody the values and principles of Health and Community through the integration into the Health Department, including Home Care, Health Benefits, Public Health and Primary Care to best serve the community and meet their health needs.

 

The Candidate:

  • Utilize Community Health nursing knowledge and expertise to promote and protect the optimal health, wellbeing and safety of infants, young children, youth, adults and families at all ages and life stages through universal and targeted programming.
  • Provide a full continuum of services, including prevention, health promotion, and early intervention services to individuals/groups in various community-based settings.
  • Collect, assess, analyze, and apply information from various data sources to make evidence-informed decisions for nursing services, including program planning, development and priority setting with individuals, families, groups, and communities, and interpret information for professional and community audiences.
  • Advocate to key stakeholders to enhance services that promote the health and wellbeing of individuals, families, groups, and communities.
  • Involve individuals, families, groups, and communities as active partners to take action to address health inequities and foster a self-management care approach for chronic conditions.
  • Advocate for and use culturally relevant and appropriate approaches when building relationships and delivering nursing services.
  • Collaborate and share knowledge with colleagues, students, First Nations, and other members of the health team.
  • Participate in collaborative, interdisciplinary, and intersectoral partnerships to enhance the health of individuals, families, groups, and populations. 
  • Perform all other tasks within the scope of the position.

 

Qualifications, Skills and Abilities:

  • Currently practicing registration as a Registered Nurse with the BC College of Nurses and Midwives (BCCNM).
  • Bachelor’s degree in nursing from a recognized university, or a Diploma in nursing from a recognized post-secondary institution or a Diploma in nursing from a recognized post-secondary institution and First Nations Health Authority (FNHA) approved training in community health (includes In-Service Training Program for NU CHN-03 levels.
  • Five (5) years' recent, related Community/Public health nursing experience, including experience related to the population applicable to the job or an equivalent combination of education, training and experience.
  • CPR certification within four (4) months of employment. Basic Life Support Certification Level C
  • Valid driver’s license, a current vehicle insurance certificate, and access to reliable transportation.
  • Current Criminal Records Check
