Clinical Pharmacist

Position Type: Admin/Professional
Department: NEOvations Pharmacy Services
Full or Part Time: Part Time
Pay Grade: BW14

Summary:

The Clinical Pharmacist is a part-time position which is responsible for the execution of medication therapy management ( MTM ) and disease state management through collaborative practice agreements ( CPA ). The Clinical Pharmacist will have the opportunity to be involved in additional services, such as seasonal immunization clinics.

Principal Functional Responsibilities:

Pharmacy Services: Deliver telehealth pharmacy services, including but not limited to:

a. Comprehensive Medication Reviews ( CMR )

b. Targeted Medication Reviews ( TMR )

c. Medication Reconciliations

d. Travel Health Consults

Miscellaneous Duties: Perform other duties as assigned.

Qualifications:
  • Doctor of Pharmacy (PharmD) degree from an ACPE accredited institution
  • Registered pharmacist in the State of Ohio with an active status
  • Active CPR license (if applicable)
  • Immunization training (if applicable)
  • Proficient with Microsoft Office Suite
Preferred Qualifications:
  • Provision of MTM services, medication reconciliations, and chronic disease management
Physical Requirements:

Must be able to utilize a phone, computer, and other office equipment.
