What You'll Do:

Provide High-Quality Care: Develop and implement comprehensive care plans for patients, ensuring continuity of care across various settings.
Coordinate Care: Collaborate with physicians, hospitals, and other healthcare providers to ensure seamless care transitions.
Educate and Support: Educate patients and families about their condition, treatment options, and self-management strategies.
Utilize Technology: Effectively utilize electronic health records and other technologies to document patient care and communicate with the care team.
Advocate for Patients: Advocate for patient needs and ensure they have access to the resources and support they require.

What You'll Bring:

Active RN License: Active RN license in HI with the ability to qualify for additional state licenses as requested.
Clinical Experience: 2+ years of nursing experience in a hospital, acute care, or direct care setting.
Renal Care Expertise: Experience in renal, chronic kidney disease, or dialysis care is essential.
Strong Communication and Interpersonal Skills: Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare providers.
Technological Proficiency: Proficient in using Microsoft Office Suite and comfortable with telehealth technologies.
